New Certification Form
Referral Guidelines
To insert a newly certifited PHT user within the USPOULTRY database and for certificate print/approval, please provide the following information:
- Name & company information for the trainer who trained the newly certified user/s.
- First and Last Name of the newly certified trainer/s. You may only submit 20 on this form.
- Company Address: Where the individual certificates will be sent to. (only one location permitted)
This new certification form is no longer in use.
Please log into the new Trainer Resources section with your valid Certified PHT Trainer Number and expiration date to access the new certification form.
